Campus Access Requirements for Students

Fall 2020 & Spring 2021

To provide assistance and instructional support, Columbia State campuses will be open to students by appointment for computer access and other designated instructional services. 


All locations – All students

The following is applicable to all students regardless of the campus they would like to access.

  • Complete the Safe Health Questionnaire and receive an “APPROVED” confirmation for campus access.
    • The online questionnaire is available on the College’s mobile app and homepage and should be completed prior to coming to campus. Completing the questionnaire prior to coming to campus will speed up the check-in process.
    • Students who do not have access to the online Safe Health Questionnaire, non-credit students, or future students may complete a paper questionnaire which is available on the website or at the campus.
    • (If you make an error in completing the questionnaire and receive a “NOT APPROVED” message when you believe it should be APPROVED – you must complete a paper questionnaire at the campus.)
  • Must have a temperature less than 100.4 to access campus.
  • Face masks are required while on campus.
  • Are asked to practice and promote the expectations of the Responsible Charger Promise.
  • Make an appointment to access campus or be a member of a class that is scheduled to meet on campus.
  • Make appointments for computer labs 24 hours prior to use. Appointments for labs are made through Navigate:
  • Check in at the specified campus check-in desk when arriving on campus. At check-in, students will receive a wristband with the date of approved access.  The wristband must be worn at all times while the student is on campus.  A new wristband is issued each day.
  • Practice social distancing of at least 6 feet.
  • Bring bottled water – Water fountains have been closed.
  • Honor signs that limit bathroom access and other campus areas.
  • The instructor/facilitator will provide instructions regarding designated locations for eating when approved campus/labs extend through the lunch/dinner period.

Computer Lab Access

Students will have limited access (by appointment only) to computers on each campus. Students must make appointments via EAB Navigate by selecting the campus and an available time on that campus. Appointments should be made at least one day prior.

Students making an appointment to access a computer lab or attend an on-campus lab must complete a Safe Health questionnaire each day before coming to campus. For computer lab access, the questionnaire should be completed before noon on the day of the visit. On the day of the visit, students with class meetings prior to 1 p.m. should complete the questionnaire by 7:30 a.m. or by noon on the day of the visit for classes meeting 1 p.m. or later.

Click here to access the online Safe Health Questionnaire. You will be required to enter your Columbia State PVT username and password to access the form.

Watch the Safe Health Questionnaire video available from the Student Tab of MyCN to learn how to complete the survey. You will only be allowed to submit the questionnaire once daily. Answer the questions honestly and accurately. Upon completion of the questionnaire, you will receive either an Acceptance message indicating you may come to campus on that day or an Access Denied message.

If accepted, make a printout or screenshot of the acceptance message on your phone and bring to campus to show faculty/staff before entering a building. If you cannot bring the acceptance message to campus, staff will be able to check your acceptance status or have you complete a new questionnaire on paper.

Pdf Format:

The purpose of this questionnaire is for you to self-observe your health prior to coming to the college campus. Take your temperature and complete the Safe Health questions every day before coming to campus. If you answer YES to questions 1-5, please stay home and let your instructor know that you cannot attend.
