- Location where services are needed (building & room number)
- Date of event
- Time that the event starts and ends
- Your phone/extension number
- Type of service requested (LCD projector, microphone, PA system, video recording, audio recording, DVD video duplication, CD audio duplication, TV studio production and/or editing, video conference via ITV system, or other type of audiovisual service).
