How do I apply?

DUAL ENROLLMENT ONLINE APPLICATION

  • Students must complete a Dual Enrollment application. The application is available in an on-line or paper format. The online format is encouraged for efficiency and ease of document tracking.
  • First time students pay a one-time $10 application processing fee. Fee payment is required before application will be reviewed. The fee is payable on-line at the end of the on-line application process, at any campus location or payment can be taken over the phone through Columbia State’s business office 931-540-2530.

On-line applications require you print the following:

  • Consent Page – The Consent form requires signatures of student, parent and high school or umbrella school.
  • Hepatitis B form – As a state funded school, Columbia State is required to provide parents and students information regarding Hepatitis B. On this document, the parental signature is what is required. Official doctors shot records are not required and cannot replace this form.

Once the application has been completed, students should request the following documents be faxed as one block of information, to Columbia State Office of Admissions, 931-560-4125.

  • High school transcripts
  •  ACT or SAT scores
  • Consent Page
  • Hepatitis B form
