Role & Responsibility

Faculty members have an important role in the delivery of approved academic adjustments for students with disabilities. 

Quickly reserve a private meeting with the student once the disability has been identified.

Provide academic adjustments in a fair and timely manner.

      Contact Disability Services immediately if there are any concerns regarding the
appropriateness of an approved academic adjustment.

Refer any student who is requesting an academic adjustment without written approval
to Disability Services. If the disability is visible, an academic adjustment is appropriate and
should be provided until the necessary paperwork is processed by Disability Services.

Approved Academic Adjustment Notification Procedure:
Students with disabilities have been informed to hand deliver their Approved Academic Adjustment form to Faculty members and arrange for a private meeting. At this meeting, students and Faculty members can discuss the approved academic adjustment and sign the Approved Academic Adjustment form. Faculty should keep a copy of the form on file and should also provide the student with a signed copy upon request.

In the case of Distance Education, RODP, and sites and centers, the Approved Academic Adjustment form may be faxed or e-mailed to the Faculty member. In this case, the Faculty member should confirm the receipt of the form with Disability Services. The student will still be responsible to self identify with the Faculty member and participate in any necessary communication with the Faculty member regarding their approved academic adjustment.

Syllabi Notice:
Columbia State Policy No. 02-12, Course Syllabi, encourages faculty to use this statement in their syllabi, “Columbia State Community College welcomes students into the college’s educational programs. If anyone in this course has a disability, please contact Disability Services in JSC 160 or call 931.540.2857.
