How to Access Services
• Identify your disability with Disability Services
• Complete forms and provide appropriate documentation of your disability
• Disability Services will review the documentation and determine eligibility
• Upon approval, an Approved Academic Adjustment form will be issued to you
• Give your approval form to the Faculty member for each class and discuss needs
• Follow-up with Disability Services to discuss any changes, problems or concerns
• Renew your academic adjustment requests each semester
• Consult the timeline for academic adjustment requests
Timeline Considerations
Contact Disability Services as early as possible as some adjustments may take considerable time to establish. Schedule changes or last minute requests may result in the delay or substitution of the academic adjustment, aid or service.
