DEVELOPMENTAL STUDIES - Forms
Several Columbia State forms are used only by the Developmental Studies Department:
- The Developmental Studies Course Withdrawal Form must be completed in addition to the institution's Change of Registration Form by all students who drop DSP courses.
- Students who wish to take courses at a level below their level of placement must complete a Developmental Studies Course Recommendation Form.
- Placement testing offices and the Columbia State Advising Center require student signatures on the Developmental Studies Information Sheet, verifying that students understand the polices governing the Columbia State placement process.
