Learning Support - Forms
Some Columbia State forms are used only by the Learning Support Department:
- Students who wish to take courses at a level below their level of placement must complete a Learning Support Course Recommendation Form.
- Placement testing offices provide students with the Learning Support Information Sheet, notifying students of the polices governing the Columbia State placement process.
- Students who wish to have their COMPASS scores sent to anther institution must complete and sign a Request for COMPASS Scores To Be Transferred.
