Although Columbia State provides all of its information online, we can provide you with printed versions of some information. Please contact admissions@columbiastate.edu or call (931) 540-2766 if you would like printed version of the following documents:

- Student Enrollment Guides
- Scholarship Applications
We will need the following information to process your request:
1. First and Last Name
2. Mailing Address - Street, City, State and Zip
3. Telephone number
4. E-mail address where we may contact you
5. Your current or previous school (high school and/or college)
6. Your birth date
